KANSAS PREVENTION "SALLY ZELLERS" LIFETIME ACHIEVEMENT AWARD
The Kansas Prevention Lifetime Achievement Award recognizes outstanding service and accomplishment spanning a career in Prevention. Nominees can be current or previous prevention professionals who have dedicated no less than 15 years to prevention efforts in Kansas.
The nomination must highlight the special or exceptional quality of the prevention professional, which makes the nominee a candidate for this Prevention Lifetime Achievement Award, and should include at a minimum: Describe how this individual has given their time and talent to Kansas prevention efforts. Describe how this individual has engaged new partners in prevention in their community or state. Describe how this person has mentored others and/or left a legacy of exemplary prevention expertise and support in Kansas.
Seth Dewey
Kansas Prevention Community Leader Award
The Kansas Prevention Community Leader Award recognizes those who have made an outstanding contribution to their community prevention efforts. Nominees can be an individual who could be described as a prevention champion.
The nomination must highlight the special or exceptional quality of the individual, which makes the nominee a candidate for this Prevention Community Leader Award, and should include at a minimum: Describe how this individual has shown leadership in their community prevention efforts. Describe how this individual leads in a way that results in others wanting to follow them. Describe how this individual has led the way for local community action and change. Describe how this individual has practiced in a way that is consistent with the factors described in the Strategic Prevention Framework.

Kansas Prevention Young Leader Award
The Kansas Young Leader Award recognizes those 22 years or younger who have shown outstanding leadership in prevention efforts in Kansas. Nominees can be an individual exemplifying young leader qualities in prevention efforts in Kansas.
The nomination must highlight the special or exceptional quality of the individual, which makes the nominee a candidate for this Prevention Young Leader Award, and should include at a minimum: Describe how this individual has shown leadership in their community prevention efforts. Describe how this individual leads in a way that results in others wanting to follow them. Describe how this individual has led the way for local community action and change. Describe how this individual has encouraged young people to support culture change needed among other young people related to healthy behaviors.

The Kansas Prevention Connected Community of the Year Award recognizes groups who have gone above and beyond in connecting their community in prevention efforts. Nominees can be communities, organizations, task forces, or coalitions in Kansas.
The nomination must highlight the special or exceptional quality of the community, organization, or coalition, which makes the nominee a candidate for this Prevention Connected Community Award, and should include at a minimum: Describe how this group has made exceptional progress connecting their community in prevention efforts. Describe how this group has increased their community readiness to change for the better. Describe how this group has increased their community capacity to change for the better. Describe how this group used their connections to others to build community resiliency, sustainability, and cultural competence.

The Kansas Partner in Prevention Award recognizes those who have made an outstanding partner for prevention efforts in their community. Nominees can be any organization, business, or any community sector entity.
The nomination must highlight the special or exceptional quality of the partner in prevention, which makes the nominee a candidate for this Partner in Prevention Award, and should include at a minimum: Describe how this partner in prevention has made exceptional progress for prevention in Kansas. Describe how this partner’s contributions may be unique and sets them apart from what may be considered common or typical prevention stakeholders. Describe the type of prevention support and resources this individual or group has provided. Summarize any known outcomes or significant outputs of the prevention-related contribution.
